Preparing for the office visit

Suggestions for your office visit

In order to make your office visit as smooth as possible, here are a few recommendations prior to comming to see me:

1)If you are a new member to The Permanente Medical Group, please obtain your medical records from your previous provider.

2)Write down your symptoms. Visit the Kaiser Permanente Health Encyclopedia at www.members.kaiserpermanente.org. Remember, the more informed you are, the better you will be able to make an educated decision to manage your health.

3)To stay healthy and prevent disease, there are few screening tests suggested by the experts. To learn which tests are encouraged, visit Prevention

4)You can either bring all the medications that you are taking, including herbal supplements and over-the-counter medications, or keep a updated list in your wallet when you come to see me.

5)If your appointment is for a complete physical exam, you may consider fasting for 12 hours prior to the visit as I may order blood test for cholesterol and diabetes. This may save you another trip back.

6) Write down your questions so they are addressed by the time you leave the office.

7) Share your thoughts of what might be causing your symptoms, so I am sure I have addressed any worries that you might have.
