Advance Health Care Directive

An Advance Health Care Directive is a very important document to let your family and doctors know how you want to be treated in case you can not make critical medical decisions yourself. California state law requires that this form be discussed with you before you come to the hospital for any surgery or medical treatment. This does not mean anything negative is going to happen.

It is strongly recommended that you fill out an Advance Health Care Directive before you have surgery. It's a complex document that you should share and discuss with your close family members. Kaiser Permanente Health Education Centers are available to help you. I have enclosed these forms here for your convenience.
