About Your Office Visit Successful visit checklist

Welcome to my website!

My website is your main access to our comprehensive online healthcare program designed for your convenience. From here you can schedule your appointments, request medication refills, send secure messages to me and access reliable health information online.

To facilitate communication, I would like to share with you some information about myself.

I grew up in Saigon, Vietnam until 1975 when my family, and I emigrated to the United States. Since then, I spent most of my time in the Bay Area. After graduating from Stanford University with a M.S.E.E. degree (Master of Science degree in Electrical Engineering), I worked as a computer engineer for six years before entering medical school at the University of California, San Francisco School of Medicine and graduated in 1996.

After completing my residency in the Internal Medicine, Primary Care, program at Santa Clara Valley Medical Center, I joined The Permanente Medical Group (at Milpitas) in July of 1999. I am a member of the American College of Physicians. I am also board certified by the American Board of Internal Medicine. I am fluent in Vietnamese.

Outside of medicine, I enjoy spending time with my wife and our son and daughter. Our son Calvin is currently in sixth grade. Besides his regular school work, he is having fun making live and stop action short videos for the Internet. Outside of school, he continues to enjoy painting (Calvin's Art Gallery), piano and tennis. Our daughter Cailin is currently in kindergarten and is very popular at school due to her outgoig manner. She enjoys talking, reading, playing games, and continues to collect stuff animals, especially cats.

I want to make certain all of your concerns are addressed. If you have more questions that were not addressed during your visit with me, you may leave a message for me at (408) 945-6183. Non-urgent messages and suggestions for improving this Web site can also be sent to me by email. Please provide your name, medical record number, and the best way to respond to you (phone, email, etc.) along with all of your messages.

An emergency medical condition is also "active labor," which means a labor when there is inadequate time for safe transfer to a Plan hospital (or designated hospital) before delivery or if a transfer poses a threat to the health of the member or unborn child.
