Welcome to my Homepage!!

I am a clinical pharmacist who practices in the Ambulatory Care and in the Chronic Care Management departments in the Hayward/Union City area. Currently, I provide care for patients in the Anticogulation Clinic, the Medication Therapy Management (MTM) Program, and the Prevent Heart Attacks and Strokes Everyday (PHASE) Clinic.

I received my Bachelor of Science degree in Molecular Biology with a minor in Chemistry from San Jose University in 2003. Then I received my Doctorate of Pharmacy degree from the University of the Pacific, Thomas J. Long School of Pharmacy and Health Sciences in 2006. I went on to do a general pharmacy practice residency at Kaiser Permanente Santa Clara Medical Center and have been with Kaiser Permanente since. I am Board Certified by the California State Board of Pharmacy.

In the Anticogulation Clinic, I manage patients who are taking warfarin, a blood thinner. I initiate the therapy when needed and adjust the warfarin doses based on INR results. In the MTM Program, I optimize drug therapy by identifying potential drug-related problems, monitoring appropriate lab parameters, and educating patients in disease states and medications. My role in the PHASE Clinic is to decrease the risk of cardiovascular complications by managing blood pressure and cholesterol.

I was born and raised in the Bay Area. I am of Vietnamese decent and am able to conversationally speak the language. In my spare time, I read novels and non-fiction books on various topics. The next book on my queue is A Long Way Gone: Memoirs of a Boy Soldier. I enjoy vacationing whether it is relaxing on the beach in the Bahamas, ziplining in Puerto Vallarta, watching Cirque du Soleil in Las Vegas, sightseeing in Europe, or best of all-snowboarding in Whistler.
