Welcome to My Internet Home Page
My name is Danita Davis and I am your clinical Complex Chronic Condition Case Manager. My program assistant is Tammie Brown. You have been referred to our Complex Chronic Conditions and it is my goal to assist you in taking control of your disease state through education, coaching and linkage to Kaiser Permanente and community resources. Your questions and concerns are important to us and we will do our best to answer them during our clinic visit and telephone visits. Depending on your needs, I call upon my colleagues to assist and advise me on delivering to you the best medical care possible. I also work with clinical health educators who will provide you with direct health education, information and counseling for conditions such as weight management, exercise, diet, diabetes care and smoking cessation.
About Me
I completed both my bachelor of arts degree and my master’s degree in social work from the California Sate University, Sacramento. I am currently working on my clinical license for social work. I have a very broad experience in life from working as a medical social worker. I have worked in dialysis, bone marrow transplant, intensive care nursery, burn unit, psychiatric unit, obstetrics/ gynecology, emergency and all other units within the hospital setting. My experience includes working at Dameron Hospital, Northside Hospital and Rockdale Hospital in Georgia as a social worker, discharge planner and utilization reviewer. I teach a social work class as an adjunct professor at University of Pacific, Stockton, CA from time to time.
I was born in Missouri, but have a Southern accent which I was born with. I have four grown children and four grandsons, Jaden, Jacob, Jack and Jonathan whom I adore. I am a Christian and enjoy growing in the Lord. If I have the time I would like to golf, go to the movies, read a good book, listen to music and travel. I am a proud member of Delta Sigma Theta Sorority and enjoy volunteer work.
My Philosophy of Care
My philosophy of care is to provide the best possible care of my abilities and to treat all patients with the same highest possible quality of care regardless of race, nationality, gender, age, attitude or economic background. I believe in speaking the truth in a humble manner and not saying what you want to hear but what I feel from the heart which is best for you. I care about your wellness more than your friendship. I strive to treat my members as how I would want to be treated and how I would want my family to be treated. I especially enjoy working with the elderly patients and their families.
Members can make a self referral appointment, or referrals can come from a variety of ways. Once a referral is made, I make every effort to address the concerns and assess the criteria for entry into the Complex Chronic Conditions Program. Entry involves either a face to face office visit or a telephone assessment. Future appointments are made for telephone follow up only.
