To take appropriate action in ensuring that you have a healthy heart, you should first determine your risk for having a heart attack or stroke. Here are some of the questions you can ask yourself to get started. We have also provided two assessment tools. The Kaiser Permanente tool focuses on heart health and the HealthMedia® Succeed™ tool assesses overall health.
• Coronary Heart Disease (Angina and Heart Attack)
• Peripheral Arterial Disease (Blocked Carotid or Femoral Artery)
• Transient Ischemic Attack (A "Mini-Stroke")
• Chronic Kidney Disease
• Abdominal Aortic Aneurysm (Enlarged Aorta)
If you do have one or more of these conditions, you are at high risk for heart attack or stroke. See our High Risk section for the information we provide to our high risk members.
Even if you don’t have a specific condition, there are several other risk factors that increase your risk for heart attack and stroke. We’ve provided two tools to help you assess your level of risk.
It will only take a few minutes to use this heart disease risk calculator to tell you your chance of having a heart attack or dying of heart disease in the next 10 years. This brief questionnaire asks about your health lifestyle, family history and medical conditions. All of these factors can affect your heart health. When you answer this short list of questions, you will receive an estimate of your 10-year risk and a set of customized recommendations to help improve your heart health and reduce your chance of getting heart disease. Your recommendations include links to information and resources tailored to your specific risks for heart disease. You can take this questionnaire and get recommendations with or without knowing your blood pressure and cholesterol level but it can only tell you your ten-year risk if you include them.

Take an in-depth look at the health choices you make each day with HealthMedia® Succeed™ and get a personal plan to improve your overall well-being and the quality of your life. You will receive recommendations for taking care of your heart and the rest of your health. This tool takes about 20 minutes to complete.
